1. CookShop Student Pre-Survey
This survey should be administered to each student BEFORE they begin the CookShop program. All responses will be private and the survey does not ask for information that can personally identify you or your student. The results of the survey will be used to assess the effectiveness of the CookShop program. For in person, you can print the survey and return to cookshop@foodbanknyc.org.
2. MyPlate: Lesson Background – Video
MyPlate is a healthy way to fill our plates. Most of the food we eat belongs to one of the 5 food groups. People need to eat foods from all five food groups and drink water every day. Eating a variety of colorful foods is one way to get the tools we need to grow strong and stay healthy!

- What types of foods do you know belong to the Vegetable group? Fruit? Grains? Dairy? Protein?
