1. CookShop Student Pre-Survey
This survey should be administered BEFORE students begin the CookShop program. All responses will be private and the survey does not ask for information that can personally identify you or your student. The results of the survey will be used to assess the effectiveness of the CookShop program.

MyPlate is a healthy way to fill our plates. Most of the food we eat belongs to one of the 5 food groups. People need to eat foods from all five food groups and drink water every day. Eating a variety of colorful foods is one way to get the tools we need to grow strong and stay healthy!
